Loot Gain and Loss?

When you attack someone and gain some loot, ie. boxer trunks, does that loot item come out of the opposing players inventory?

Questions on PvP loot:

Is it completely random when and what you get awarded for winning a fight?

Does the "power" of the opposing player factor in to anything?
